HOW TO BECOME A MEMBER
1. Complete the on-line application.
2. Pay for your membership.
3. Print the Medical Release Form.
4. Take the HIV test at your preferred location.
5. Return proof of HIV test results and the Medical Release Form, copy of drivers license or picture ID, and verification of address.
6. Information will be entered on HIV FREE check member status page.
7. HIV FREE will mail your HIV FREE membership card within 4 to 6 weeks.
